Thank you for participating in the Make a Wish Giveaway. Simply complete this form and deposit at point of promotion. Multiple entries and unreadable entry forms will be disqualified. Awarded gifts and prizes may not be exchanged for cash. You must be at least eighteen years old in order to be awarded any prizes; however prizes may be awarded to a parent meeting certain requirements provided their name(s) are included on the entry forms. The odds of winning any particular prize will depend upon the number of entries received. Winner will have choice of two hundred movies passes, on thousand gallons of gas, 65” Big-Screen TV, or $2,500 in cash. Pacific Monarch Resorts (PMR) will award the choice of a prize to one winner as determined by a random drawing, which will be held on December 31,2005 at PMR's corporate office. The two hundred movies passes will be in the form of Regal Entertainment Group's “Premiere” movie tickets, which may be used at various Regal movie theatres, including Edwards Theatres, Regal Cinemas and United Artist Theaters. The movie passes are unrestricted and are readable for any movie and during any show time, including opening night. The movie passes may not be used for the purchase of items such as popcorn, candy, beverages, T-shirts or any other food or merchandise. The one thousand gallons of gasoline will be in the form of a gift card, which may be used only at the gasoline/service station of the gift card issuer. Use of the gift card for purchase of items other than gasoline is subject to issuer rules and restrictions. The Big-Screen TV will be selected solely by PMR. The winner of any gift under this promotion (including cash) will be solely responsible for payment of any and all taxes(including any taxes related to taxable income). In the event PMR is unable to contact the winner and/or accept delivery of a prize within fourteen (14) days of the drawing, PMR will conduct one random drawing on or about the 15th day of each subsequent month until one prize has been awarded, accepted and delivered. Acceptance of any gift or prize constitutes your permission for PMR's use of your name and likenesses for promotional purposes without further compensation except where prohibited by law. The sponsor of this promotion is Pacific Monarch Resorts, Inc., whose business is vacation timeshare resort ownership. This giveaway is not promoted, sponsored and/or administrated by any shopping mall or movie theatre. This promotion is valid only in the states of California, Utah and Nevada. Prizes cannot be awarded via telephone or the Internet.
